Hair Loss Study

First, THANK YOU for participating in this important research project! The samples you provide will allow researchers to continue discovering the genes controlling traits in our breed, and dogs in general. As the canine genome is mapped, breeders will have an unprecedented opportunity to identify and avoid producing disease, and concentrate on positive advances in their breeding programs.

If you are unsure about what samples will be useful for research, please view the Useful Research Families page.

The forms are available for viewing in HTML format. If you are going to submit these forms, we prefer that you use either the Microsoft Word v8.0 (Office 97) or PDF (Adobe Acrobat) format to print them. Each form is designed to fit on one page, although it may run over to a second page. It should appear as you see it here on the screen - if it prints with wide spaces, disconnecting the various parts, please try a different format to print. Thank you!

HTML Documents (view only-DO NOT PRINT from here - the forms probably will not be correct! See above)

Please use your "back" button to return to this page after viewing the form.
